PAYMENT $50 nonrefundable deposit and supply fee are due at the time of registration or you may pay in full. Balances are due 7 days prior to the first day of camp. Full payment includes a $50 nonrefundable deposit. In the event of a cancellation, the remainder of your payment minus the $50 deposit will be refunded up to 7 days prior to the start of camp. Within 7 days of the camp start date your payment is non-refundable. Cancellations must be received in writing, via email, up to 7 days prior to the first day of camp. Cancellations within 7 days of the camp start date will not be refunded or credited towards another camp or class. Additionally, any days missed during the camp week, even in the event of unexpected illness, will not be refunded and make up classes are not offered.
RELEASE OF LIABILITY I hereby release and agree to indemnity and hold harmless Michelle Giddens Art, Michelle Giddens, St. Michael Catholic School, St. Michael Catholic Church and the Archdiocese of Mobile from any and all losses, injury, illness, including COVID-19, damages, liabilities, claims, costs, and expenses (including attorney fees) which my child may sustain or incur in any way arising out of or in connection with the child's participation in any and all camp activities.
MEDICAL In the event of a medical emergency, I hereby give permission to Michelle Giddens and teachers, Inc. and/or medical personnel to secure, at my expense, proper treatment as necessary, for the child(ren) named on the Registration Form. I understand that all reasonable efforts to contact me will be made should an emergency arise.
